ABSTRACT

Objective:For severe skin defects which are deep to dermis, engineered skin with epidermis and dermis (bilayered) is required. Based on the success of engineering epidermis with GT/PCL electrospun membranes, our study was to investigate whether this membrane could be also used for engineering bilayered skin graft.Methods:From 2013 to 2019, we first prepared three GT/PCL electrospun membranes with different proportion (70∶30; 50∶50; 30∶70) in our laboratory; the biocompatibility of the membrane was evaluated in vitro by seeding fibroblasts or keratinocytes on the membranes. Then the outcome of GT/PCL membranes repairing skin defects in the nude mouse was investigated.Results:Cell attachment and proliferation were significantly improved with increase of gelatin. Histological analyses showed that bilayered skin engineered with GT/PCL (70∶30) group could form relatively better structure after 3 weeks of cultivation in vitro. Further in vivo transplantation studies revealed that scaffolds were not degraded in all three groups, indicating that these materials were not suitable for engineering bilayered skin although they had good biocompatibility.Conclusions:The higher gelatin membranes possess better biocompatibility. Further in vivo transplantation studies reveal that bilayered skin engineered with GT/PCL membranes is able to repair skin defects in the nude mouse.

ABSTRACT

Objective To study the relationship between the quality of Astragalus membranaceus and soil in Gansu Minle. Methods we sampled the medicinal materials and soils of Astragalus membranaceus in Minle County, and determined the medicinal quality, soil factors and heavy metal elements by high performance liquid chromatography, ultraviolet spectrophotometry, atomic absorption spectrophotometry, and etc. Results The quality of Minle Astragalus meets the requirements of the Pharmacopoeia standards and is low on the risk of heavy metal pollution. There was a significant negative correlation between the extract and the available potassium. There was a significant positive correlation between the astragaloside IV and the available potassium. There was a highly significant positive correlation between calycosin-7-glucoside and soluble salt. There was a significant positive correlation with available potassium, and a significant negative correlation with pH. Among them, soil pH value is the most important factor affecting the content of astragalus extract, astragaloside IV and verrucous glucoside, followed by total phosphorus, organic matter and total nitrogen. Conclusion The results revealed that the soil factors is the main influencing factor of the quality of Astragalus membranaceus in Minle County, which can provide a theoretical basis for the standardized cultivation of Astragalus membranaceus in Minle County.

ABSTRACT

Objective:To study the mechanism of free bone graft survival in calvarial defects.Methods:We simulated the bone autograft reconstruction through transplanting green fluorescent protein (GFP) mice' cranium to the nude mice' calvarial bone defects. Then gross and histological evaluation were performed to calculate BTA (%) and GFP (%) in graft bone.Results:The results showed that the green fluorescence was getting disappeared from 4 to 24 weeks' post-operation. However, a small amount of green fluorescence remained at 24 weeks' post-operation (6.69% of the positive control group). In addition, the gross and histological evaluation indicated that the grafted bone survived well at different time points.Conclusions:The survival mechanism of free bone graft in cranium mainly relies on creeping substitution assisted with bony regeneration.

ABSTRACT

Objective:To measure the changes in the orbital and nasal parameters before and after Tessier box osteotomy with primary aesthetic rhinoplasty concurrently with byotogenous costochondral graft.Methods:From 2013 to 2018, 60 hypertelorism patients were treated in our craniofacial center. Correction involved a classic Tessier box osteotomy combined with aesthetic augmentation and reconstruction of the nasal dorsum using a costochondral graft. All patients underwent photographic and tomographic documentation both before and 12 months after surgery. Patients underwent morphometric analysis to document the changes in interorbital distance (IOD) and aesthetic nasal parameters, including the nasal dorsum and alar width, the nasal dorsal height, and the index of nasal apex protrusion versus nasal length.Results:All patients were satisfied with the outcome of the combined orbital and nasal repair. Morphometric analysis indicated that the IOD and aesthetic nasal parameters all improved following surgery. The resorption rate of the costochondral graft was 16-19%, and graft warpage was nearly absent. Although complications so as infection, cerebrospinal fluid leak or hyposomia, no severe or life-threatening complications occurred.Conclusions:Tessier box osteotomy with primary aesthetic rhinoplasty using costochondral grafts has showed to be efficacious in the treatment of patients with hypertelorism.

ABSTRACT

Objective To explore a new method for the treatment of nasal deformity in the middle part of facial cleft.Methods Eight patients with craniofacial cleft midface depression were treated with nasal reconstruction,nasal reconstruction plus pear shaped hole week osteotomy forward,pyriform aperture osteotomy advancement by using the auxiliary week preoperative design and postoperative surgical navigation system,and the preoperative design and postoperative CT data fit was compared.Results All the 8 patients were satisfied with results of the operation.There was no significant difference in CT data between preoperative simulation design and postoperative follow-up.Conclusions The expanded forehead flap in nasal reconstruction surgery has good effect on congenital craniofacial cleft nasal defect,and surgical navigation assisted pyriform hole osteotomy advancement for correction of congenital craniofacial cleft caused by midface depression to achieve a precise osteotomy,has good application value.

Objective@#To observe the influence of lower wide pedicle frontal periosteum flap on frontal bone absorption rate after cranioplasty.@*Methods@#From February 2016 to July 2017, the lower wide pedicle frontal periosteum flap was produced in 12 patients of Shanghai Ninth People′s Hospital (7 males/5 females, aged 5-9 years, 10 hypertelorism, 2 Crouzon syndrome), who accepted intro-cranio-route plastic surgery, to cover the frontal bone window. A spiral CT scans were applied one week (t1) and one year (t2) after surgery. DICOM data was imported into Mimics software to reconstruct the 3D model of skull. The bone window covered the frontal bone was selected and the bone volume was calculated. The absorption rate was calculated as (Vt1-Vt2)/Vt1×100%. As a control group, the CT data of 20 patients (from January 2010 to December 2015, 11 males/9 females, 7 hypertelorism, 12 Crouzon syndrome, 1 Pfeiffer syndrome) were analyzed retrospectively in the same way, and compared to the experimental group.@*Results@#The average bone absorption rate in experimental group was 8.65%±2.56% (n=12), while in control group it was 26.51%±5.23% (n=20). Significant statistical difference was observed. No further cranial defect was observed in one-year follow-up in both two groups.@*Conclusions@#The lower wide pedicle frontal periosteum flap reduces the bone absorption rate after intro-cranio-route plastic surgery. It could also help to repair the anterior cranial base defect during the surgery. This technique was recommend as a regular step in craniofacial surgeries.

ABSTRACT

Objective To investigate the contouring plastic effect of the digital navigation techniques assisted mandibular angle osteotomy,thereby providing surgical reference and evaluation basis for improving the accuracy,efficacy and safety of surgery.Methods From March 2015 to March 2017,15 female cases of the digital navigation aided intral-oral mandibular angle osteotomy in the past 2 years were collected and compared to the pre-operational image and digital design.Results The postoperative effect of navigation-assisted mandibular angle osteotomy was well matched to that of preoperative design.The mandibular angle was smooth and bilateral symmetrical with no obvious complication occurred.Conclusions Navigation-assisted techniques can solve the problem of limited exposure of intra-oral approach and significantly improve the accuracy,effectiveness and safety in mandibular angle osteotomy.

ABSTRACT

Objective To evaluate the etiology,therapeutic approaches and prognosis of orbital hypertelorism.Methods Clinical data of 23 cases were summarized with review ot the literature.Results All operations provided satisfactory cosmetic effects.All the patients were followed up for 6 months to 5 years,and no recurrence were observed.Conclusions A good cosmetic effect can be obtained by operation in orbital hypertelorism.The preschool period the best opportunity,and stable effects could be gotten and the psychological development of children has not been influenced.The intracranial and extracranial joint pathway orbit box osteotomy still has drawbacks and requires to be improved.The etiology of gene and molecular aspects need to be explored.
